ASSUMPTION OF RISK
I, the undersigned participant (or parent/legal guardian if participant is a minor), acknowledge that participation in the High Point Showcase, organized by High Point Media Collective, LLC, involves physical activity and inherent risks including, but not limited to, physical injury, sprains, fractures, and in extreme cases permanent disability or death. I understand that these risks cannot be fully eliminated. I voluntarily assume all risks associated with participation in this event, whether known or unknown.
RELEASE OF LIABILITY & WAIVER
In consideration of being permitted to participate in the High Point Showcase, I, on behalf of myself and my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue High Point Media Collective, LLC, its Co-Founders, employees, volunteers, agents, event staff, and affiliated venues (collectively, "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained during participation in this event, whether caused by the negligence of the Released Parties or otherwise.
INDEMNIFICATION
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur due to my participation in this event.
MEDICAL AUTHORIZATION
In the event of an emergency, I authorize the Released Parties to seek and consent to medical treatment on behalf of the participant if a parent or guardian cannot be immediately reached. I understand that I am responsible for any medical expenses incurred.
PHOTO & MEDIA RELEASE
I grant High Point Media Collective, LLC permission to photograph and/or record video of the participant during the High Point Showcase for use on social media, promotional materials, and related platforms. No compensation will be provided for such use.
ACKNOWLEDGMENT
I certify that I have read this document in its entirety, that I understand its contents, and that I am signing it freely and voluntarily. If the participant is a minor, I represent that I am the parent or legal guardian and have the authority to execute this agreement on their behalf. NOTE: This waiver is governed by the laws of the State of Florida.
